Confidentiality is at the core of high-quality patient care. Sunrise Clinics takes your right to privacy and confidentiality very seriously and has set up a number of mechanisms to assure that the confidentiality of patient information is protected. In addition, we want to:

- respect your rights as a patient

Communications Preferences
During the new patient registration process, the Health Center collects contact information such as telephone, email, and cell phone numbers for the purpose of sending reminder calls, patient satisfaction surveys, and other matters related to your health care. Patients may opt-out of all contact by completing the request form during registration or may request one from their care team any time after.
