Medical Records
Although your medical record data belongs to Sunrise Clinics, the information contained in the record belongs to you. You may inspect the records or have copies made for a reasonable fee after signing an authorization.
To request copies of your medical record, download and fill out the Authorization to Release Medical Record and fax it to your clinic.
Sunrise Clinics has a patient portal that does have some components of your medical information available for your review. If you have not enrolled, you can call your clinic to be set up in the Portal.
At Sunrise Clinics, we respect your privacy. If you would like us to forward a copy of your medical record to a healthcare provider, we need your written authorization along with a legible copy of your valid photo ID. If you pick up your medical record in person, you will be required to show a valid photo ID.
There are 3 options to obtain copies of your medical records:
- Stop by any of our clinics and fill out an Authorization to Release Medical Record. These requests will be forwarded to the Medical Records Department to process. The form is also available in Spanish.
- Download and complete the Authorization to Release Medical Record form and mail it to your clinic.
- Download and fax Authorization to Release Medical Record to your clinic.

Is there a charge for medical records?
There is NO charge if we send the records directly to another health care provider.
There is a charge for copies of medical records you pick up or those which are mailed directly to you. This fee is legislated by the State of New Mexico. Payment for copies can be made by cash, check or credit card.
