School Based Health Centers
Sunrise Clinics school-based health centers offer health services for children and teens on-site at their schools as well as faculty and staff. Health care is provided for those who have a primary care provider in the community as well as for those who use the school-based health center for their primary care. When school is not open, services are available at our health center sites in Santa Rosa, Tucumcari, and Las Vegas for these students.
Services include:
- Medical examinations
- Health assessments and education
- Support for Social Determinants of Health
- Sports Medicine, including sports physicals
- Treatment for acute and chronic illnesses
- Behavioral health
Students must be enrolled in the school-based health center to receive services. Forms can be obtained at the school-based health center or downloaded below. They must be signed by a parent or guardian and returned to the clinic for enrollment.
