Payment for Services

No one will be denied access to services due to inability to pay. If you are not eligible for insurance coverage, you may still qualify for a discounted fee based on your family size and income.

Private insurance, Medicare, Medicaid and self-pay are accepted for all services.

Sliding Fee Discount Schedule

Sunrise Clinics offers a discounted fee for patients and their families who fall at or below 200% of the Federal Poverty Level. A discounted fee is determined based on the size of your family and your family income. Family includes you, your spouse, and your dependents under the age of 18.
