Our sliding fee clinic is part of a federal program that allows us to offer discounted healthcare services based on your income. This sliding fee clinic discount is available to all eligible patients, regardless of their insurance status. At Sunrise Clinics, we will never deny healthcare due to an inability to pay. The sliding scale fee means that the more financial need you have, the less you pay for your visit, making healthcare affordable for all.
Services Covered by the Sliding Fee Clinic
The sliding fee clinic applies to a wide range of services provided directly by Sunrise Clinics. This includes medical visits, behavioral health visits, lab tests, physical therapy, and more. Please note that services provided through referrals are not covered by the sliding fee clinic program, but we will do everything we can to make sure you receive the care you need.
How to Qualify for the Sliding Fee Clinic
Eligibility for the sliding fee clinic program is determined by your household size and annual income. To apply, you’ll need to fill out an application and provide proof of income. Acceptable documentation includes tax forms, pay stubs, social security statements, and other relevant documents. If you are self-employed, your net income will be used to determine eligibility for the sliding fee clinic discount.
How to Apply for the Sliding Fee Clinic Program
Applying for the sliding fee clinic program is easy! Simply visit our front desk to request a sliding fee clinic packet. Our staff is available to answer any questions and help you fill out the application. Once submitted, you’ll need to provide proof of income within 30 days to complete your application.
Proof of Income for the Sliding Fee Clinic
To apply for the sliding fee clinic discount, you’ll need to provide proof of income for everyone in your household over the age of 18. Acceptable documents include:
- Most recent 1040 tax form or W-2’s
- Recent pay stubs or award letters from Social Security
- Bank statements showing income deposits
- If you have no income, a self-declaration form is available

Who is Considered a Household Member?
For the purposes of the sliding fee clinic program, household members are those related by blood, marriage, or adoption. This includes anyone financially responsible for one another. Please note that roommates do not qualify as household members.
When Does the Sliding Fee Clinic Discount Expire?
The sliding fee clinic discount is valid for one year from the date of approval. To continue receiving benefits, you must reapply before the expiration date. We’ll remind you when it’s time to renew, so you can continue to benefit from affordable care.
What Happens if I Don’t Want to Share Financial Information?
While we understand that some people may hesitate to provide financial information, eligibility for the sliding fee clinic discount is based on income. If you choose not to share your financial details, you’ll be responsible for the full cost of services. If you’re unsure whether you qualify, simply mark "X" on the registration form, and our front desk staff will provide assistance and answer any questions.
